About the program

VA Grant and Per Diem (GPD) Case Management

The Michigan Veterans Foundation has been awarded a GPD Case Management Grant from the U.S. Department of Veterans Affairs. This program provides a 6-month continuum of care focused on housing stability and independent living — at no cost to the veteran.

The program serves veterans experiencing homelessness who are approximately 30 days from being housed, as well as veterans in permanent housing who need support to maintain stability.

    What is the GPD Case Management Program?

    The VA Grant and Per Diem (GPD) Case Management Program is a federally funded initiative through the U.S. Department of Veterans Affairs. MVF has been awarded this grant to provide up to 6 months of intensive, personalized housing stabilization support to veterans in Southeast Michigan — at no cost to the veteran.

    Who is eligible for the program?

    U.S. military veterans 18 or older who are experiencing homelessness or at risk of homelessness, have a discharge status of Honorable, General Under Honorable, or Under Other Than Honorable, and are not currently receiving case management services through another VA-funded program (such as HUD-VASH or SSVF).

    How long does the program last?

    Eligible veterans may receive up to six months of case management services. Support is personalized and flexible based on each veteran’s needs and goals.

    What is the $500 incentive?

    After 30 days of active participation in the program, veterans will receive $500 in incentive funds to assist with essential needs such as clothing, groceries, furniture, transportation, or household items.

    Can I refer a veteran I am working with?

    Yes. Case managers, social workers, and organizations are welcome to submit referrals on behalf of veterans. Select ‘Organization referral’ in the referral form and complete the referring person section. You can also email bklovski@mvfdetroit.org or call (313) 831-5500.

    How long does it take to hear back after submitting a referral?

    A GPD case manager will contact the veteran or referring party within 3 business days of receiving the referral. For urgent situations, please call us directly at (313) 831-5500.

    Is my information kept private?

    Yes. All information submitted is treated as Protected Health Information (PHI) under HIPAA. Your data is used solely to determine eligibility, provide services, and meet program reporting requirements. It will not be shared without consent except as required by law or to provide direct services.
